You need to save Survival Instinct for 3rd and 4th annihilate unless you have some other trinket. Sometimes they'll just hover around yellow threat levels and refuse to get off Velen until you CC or burst them with damage. Any kind of self-healing should help aggro them, but there's no guarantee. P2 is all about praying adds/infernals don't cling to NPCs while you're circle-kiting the boss to avoid jump puddles being placed on orbs. They need to die regardless, and as long as boss is dotted you can focus adds down. You still want to use it for adds, but you want to be just outside P1 degen to clip the boss with it for extra damage.
You can try to time your Barkskin to burn the adds, but it will go offsync as P1 progresses. Don't bother using stuns/knockback unless you're desperate and trying to get to an orb. Wait till arcane adds finish their cast and immediately use disorient roar (it auto-puts you in bear form, no need to pre-shape) to interrupt otherwise they will start recasting. If you get 3 infernals just leave the instance and save yourself a repair bill. You don't ever need to stand in degen unless you're regular interrupting boss. Use Typhoon to interrupt first and every other Drain, while normal interrupt or orb to interrupt the rest.
Infernals are optional, but if you can't sit an cast and boss has sunfire+moonfire on it already, dot them up anyway.
